Program Team Photo Template  *
Please down load this Form and save it as "Your Team Name's Photo".  Click once in the box that says "Enter Team Name here" * Right click on the Photo, select "Change Picture" and replace with your team's picture. Next click once in the box that says "Player/Staff Names" Please type your players Names followed by the Staff's positions & Names.     NO ALL CAPS - NO POSITIONS or other comments PLEASE.  Save the changes and email file to Teampics4lv@aol.com by July 15th. 

College Book Form    * Please download this form.  Fill In all the information, save it as "Your Team's Name" and email it to Diamondgirls09@yahoo.com    The deadline to have your team in the College Book is July 15th.

Gate Pass Pre Order Form  ** This Form is for a Team Pre Order Only.  The Gate Passes will be given to the Manager at Team Check In.  We will be offering Tournament Gate passes at Team Check In also for those families that were unable to do a Team Pre Order.

Manager/Coach Meeting/Breakfast Confirmation Form  * It is mandatory that one Staff member from each team attend the Mgr/Coach Meeting/Breakfast.  Please submit this form to confirm your attendance and to purchase additional Staff Tickets if needed.  Deadline is July 15th.

Practice Time/Scrimmage Game Request Form  * Please complete request Form and mail with payment by July 15th.

Souvenir Pre Order Form  **  Deadline has been extended to July 20th   *  you can download this form and mail it with payment or there is a link at the bottom that allows you to pay online and then email the order to SallyASA@aol.com.

The Following Forms must be recieved by the Tournament Director by July 15, 2009 to be offically entered for this event:

Tournament Payment Form  * Fill out completely & mail with payment

Official Tournament Entry Form  * Must be signed by Association Commissioner, his/her designee or Qualifying Tournament Director

Championship Roster  * Must be signed by Association Comissioner or his/her designee

Championship Play Requirements Form   * Must be signed by Association Comissioner or his/her designee

The Pick Up Player Form can be mailed in or turned in at Team Check In

Pick Up Player Form  * Must be signed by Association Comissioner or his/her designee


Tournament Check In Form may be prepared in advance by filling in Player's Names & birthdates in the exact same order they are listed on the Championship Roster.  List Pick Up Players at the bottom.  DO NOT HAVE PLAYERS PRE SIGN - SIGNATURE MUST BE WITNESSED AT THE CHECK IN TABLE.
